The two-year window that decides whether bariatric surgery works

Weight regain is a follow-up problem, not a surgical one. What a proper 24-month programme looks like, month by month.

Bariatric surgery reliably produces weight loss in the first year. Whether that loss is still there at five years is decided almost entirely by what happens in the twenty-four months after the operation — nutrition, supplementation, behavioural support and honest monitoring.

A structured programme looks unglamorous: protein and micronutrient targets from week one, dietitian contact at defined intervals rather than on request, psychological support available by default, and blood panels at three, six, twelve and twenty-four months to catch deficiencies before they become symptoms.

This is why we treat aftercare as part of the operation rather than an add-on. A package that ends at the airport is not cheaper — it simply moves the cost of the missing two years onto the patient, and usually onto their own health service.
