Ultrasound-guided BBL: what changed after the 2024 safety consensus
Subcutaneous-only grafting under live ultrasound has moved the mortality figure by an order of magnitude. What to ask your surgeon before you book.

Fat transfer to the buttock earned its reputation for risk from one specific mechanism: fat entering a gluteal vein and travelling to the lung. The variable that governs that risk is depth. Grafting confined to the subcutaneous plane, above the muscle, removes contact with the deep vessels where the mechanism begins.
Live ultrasound guidance is what turns that principle into practice. Rather than inferring cannula depth from feel, the surgeon sees the plane on screen throughout, and the anaesthetic team monitors for the earliest signs of embolism rather than waiting for late ones.
Before booking anywhere, ask three questions and expect specific answers: is grafting exclusively subcutaneous, is ultrasound used intraoperatively rather than only for assessment, and what volume is planned per side. A surgeon who cannot answer all three precisely is not the surgeon for this operation.
